We are located across the street from the newly renovated International Market Place in Waikiki. Street-side parking is available as well as many parking lots one to two blocks away. We are situated on the second floor of the Kuhio Banyan Hotel above the Surf N Turf Taco shop. Stairs are located on the corner of Nahua and Kuhio. An elevator is located in the front lobby!
We perform COVID testing for general screening as well as pre and post travel requirements. There is usually no wait for our tests and appointments are not necessary. Simply walk in, fill out a registration form, get swabbed and leave. It’s that simple. Test results are emailed in under an hour in PDF format to submit to official parties. Our state of the art PCR screener results are accepted by the state, airlines, restaurants, employers as well as Canadian, Japanese, Thai, and European international travel officials. The test performed is a PCR (Polymerase Chain Reaction) also known as the NAAT (Nucleic Acid Amplification Test) and is used to identify the antigen (or virus). Cash price for asymptomatic COVID testing is $135.00 plus tax. Please call our office for any questions. Many insurers pay for these tests so please check with your insurance.
We perform the gold standard Rapid PCR antigen tests which gets you back home. We have assisted lots of travelers and our tests are accepted by all of the airlines. If you do test positive and have to isolate some airlines require a “clearance letter” which we can provide. We also have the forms required to fly home (after testing positive and isolating) for QUANTAS Airlines. No appointment is necessary. If you have any questions please give us a call.

We understand the headache and anxiety associated with the immigration process. We make the procedure very easy. Immigration physicals have been performed here for more than 30 years. No appointment is necessary. Please just walk in Monday thru Saturday.
What you’ll need:
1) A government issued photo identification (a passport is best but not necessary).
2) Copies of any/all vaccination records. Covid-19 vaccination is now a requirement.
3) A list of your medicines if you are taking any.
4) A smile.
What we do:
1) We use computerized software to accurately and completely fill out the i693 form FOR YOU. Copies are kept in our computers in case you need additional copies emailed to you later.
2) We perform a blood test for syphilis (RPR) as required for anyone aged 18 - 44 year olds. Results are returned from the lab within 24 hours.
3) We perform a urine test for gonorrhea as required for anyone aged 18 - 24. Results are returned from the lab within 72 hours.
4) A clearance for tuberculosis is required within the previous 12 calendar months. Beginning in October of 2018 the Department of Immigration switched from the ”TB skin test” to a blood test. This blood test takes 3 - 7 days. If the TB blood test is positive a chest x-ray to screen for tuberculosis will need to be performed unless there are reasons to avoid x-rays (e.g. pregnancy). Chest x-rays can be performed at a nearby radiology clinic usually on the same day.
5) We administer the vaccines required by the department of immigration which include:
a) Tetanus toxoid. A tetanus booster must be documented within the previous 10 years.
b) Hepatitis B vaccine. If none is documented then one can be given in the clinic.
c) MMR. Measles (actually measles-mumps- rubella combination) vaccination must be documented. If none are documented then one can be given in the clinic.
d) For persons 65 years and older the department of immigration requires immunization for pneumonia. We do not administer this vaccine but it can be given the same day at a vaccine clinic nearby.
e) Children’s vaccinations are performed for free if they are not up to date. Please check this website to see if your child’s vaccinations are up to date: https://www2a.cdc.gov/vaccines/childquiz/
The department of health sponsors the VFC (Vaccines For Children) program https://www.cdc.gov/features/vfcprogram/ which supplies participating clinics with free vaccines for children under the age of 18.
Please bring in all children’s vaccination records. If you believe that your children are not up to date with their vaccines then you may make arrangements beforehand by calling the Waikiki Health Center at 808-922- 4787. They participate in the VFC program and have two offices (one is located in Waikiki six blocks from our clinic). We do not vaccinate persons under the age of 18.
f) During the flu season (Oct. 1 thru March 31) an influenza vaccination must be documented. If no flu documentation is available then we are able to vaccinate you.
TIMING
It usually takes 4-7 business days for immigration physicals. This time frame is required in order to perform the required TB (blood) & gonorrhea (urine) test.
Some people have all of their vaccinations performed beforehand as well as tests for tuberculosis, gonorrhea, and syphilis. In these cases the immigration physical is done the same day, paperwork finished, and the entire process finished in 1 day.

We offer additional services to employers including pre-employment examinations and instant drug screening 6 days a week. Results of drug screens are called back to the employer, same day, and an email confirmation is sent as well. When we say instant we mean instant.
